In-Home Support

Many older adults and people with disabilities prefer to live in the homes and communities of their choice. Supports and services can help you stay safer at home, delaying dependency and the need for more restrictive and expensive living and care arrangements.

Senior Care

Senior Care provides case management and support services for people age 65 years or older who are at risk of nursing home placement. Eligible adults may qualify for in-home help with personal care, chores, medications, medical equipment and supplies, adult day care, respite care, home-delivered meals, transportation, and emergency response systems.​​

Home and Community-Based Services

Maryland Medicaid offers home and community-based services for older adults, people with disabilities, and children with chronic health conditions. Eligibility criteria and benefits vary for each program, but can include in-home supports with activities of daily living, adult day programs, transportation assistance, and other home and community-based supports.

Community Supports for People with Developmental Disabilities

The Community Supports Waiver helps people with developmental disabilities live more independently by providing meaningful supports that promote community living, such as employment assistance, home modification assessments and resources, daycare, housing and transportation support, mentoring, and case management, and other services and benefits.

Senior Call Check and Social Connections

Senior Call Check and Social Connections provides peace of mind to families and caregivers of older adults who live alone by providing daily check-in services. If the program participant cannot be reached, notifications go out to a designated emergency contact. The service is free.

Long-Term Caregiving

Maryland Medicaid programs and waivers provide community services options as an alternative or a delay to accessing care in a nursing home. Programs offer support with activities of daily living, nursing assistance, and specialized medical care to help program participants remain at home.

Medical Equipment and Assistive Technology

The right equipment can help older adults and people with disabilities stay safer in their homes and communities. Maryland Equips offers free medical equipment and assistive technology without the need for insurance or a prescription. The service is available to Marylanders of all ages.
