Information for Providers

Community-based organizations, health care, and social service providers can benefit from partnering with MAP. If you provide a service or resource that supports older adults and people with disabilities, we encourage you to register your organization to be part of the MAP referral system.

Why Register with MAP?

MAP saves service providers time and money by answering benefit questions from callers, finding the programs and services they qualify for, and helping them gather necessary documentation. Then we send only eligible referrals your way with a better understanding of the program and application process so you can focus on serving clients.

Who should Register with MAP?

State and Local Government

Health and human services and other resources provided by state and local governments are constantly evolving. It's important to register new programs and update changes to existing programs to ensure they are accessible to those who qualify.

Nonprofits and Community Based Organizations

Programs and services administered by organizations at the local level are essential to the wellbeing of older adults and people with disabilties. Registering your organization with MAP can help more community find the support they need.

Medical and Care Providers

Caregivers, hospitals, rehabilitation centers, and other medical and health care providers provide the foundation on which long-term care services are built. MAP can connect these critical providers to those who need them, and build a stronger continuum of care with other essential services at the local level.

MAP Services

  • In-home and community-based supports including caregiving, welfare check-ins, and adult day care services.
  • Social and educational programs including personal enrichment programs and community connection events.
  • Food access including grocery and meal distribution, congregate dining programs, and support for accessing SNAP benefits.
  • Health services including medical providers and resources, and accessing coverage and support.
  • Housing supports including assisted living, nursing home, and community housing programs, and rent, utility, and home modification supports.
  • Financial assistance including social security, disability, and tax credit services.
  • Transportation for older adults people with disabilities.
  • Legal and advocacy services and supports to help older adults and people with disabilities protect their rights and prevent against exploitation and abuse.
