Being prepared for your Options Counseling session can ensure that all your needs and wishes are addressed and help expedite the process. The following checklist can help you prepare for your session with a local MAP Counselor.

  • Whether or not you are a veteran

  • Any caregivers, providers, or powers of attorney that help with your finances or care decisions

Who should you speak with before your counseling session?

  • A spouse, family, or friend who can help you prepare
  • A caregiver, provider, or power of attorney who can provide important information

What should you bring to the counseling session?

For example:

  • A list of any programs you are part of or public benefits you receive
  • Identification (state I.D. or passport)
  • Proof of income and assets
  • Proof of military service or veteran's status
